29 CFR 1926.100(a): Employees working in areas where there was a possible danger of head injury from impact, or falling or flying objects, or from electrical shock and burns, were not protected by protective helmets:   (a)  Jobsite:  On or about September 2, 2014, the ground man was not wearing a hard hat.  There were four employees working at an elevated height above the ground man and they were all using hand tools.

29 CFR 1926.451(g)(1)(i): Each employee on a boatswains' chair, catenary scaffold, float scaffold, needle beam scaffold, or ladder jack scaffold was not protected by a personal fall arrest system:    (a) Jobsite:  On or about September 2, 2014, two employees were working form a ladder jack scaffold and they were not protected from falling to the ground.  The fall hazard to the two employees was about 12 feet to 14  feet.

29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet (1.8 m)  or more above lower levels was not protected by guardrail systems, safety net systems, personal fall arrest systems, or an alternative fall protection measure under another provision of paragraph 1926.501 (b), nor did the employer demonstrate that it is infeasible or creates a greater hazard to use these systems and develop and implement a fall protection plan which meets the requirements of paragraph (k) of section 1926.502:    (a)  Jobsite:  On or about September 2, 2014, two employees were working from a roof and they were not protected from falling to the ground.  The fall hazard to the employees ranged from about 14 to 16 feet.

29 CFR 1926.503(c): When the employer has reason to believe that any affected employee who has already been trained does not have the understanding and skill required by paragraph (a) of this section, the employer is to retrain each such employee such as inadequacies in an affected employees knowledge or use of fall protection systems or equipment indicate that the employee has not retained the requisite understanding of skill:    (a)  Jobsite:  On or about September 2, 2014, the employer did not retrain his employees in the knowledge and use of fall protection equipment when working from a roof or ladder jack scaffolding.
