29 CFR 1926.451(b)(1): Each platform on all working levels of scaffolds was not fully planked or decked between the front uprights and the guardrail supports as specified in paragraphs 1926.451(b)(1)(i)-(ii) a) South west corner - employees were exposed to a fall of 10 feet 6 inches while working on a form scaffold. The scaffold bracket was not fully planked in that the bracket was 33 inches wide with 2 wood planks which were 7 1/2 inches wide each, on or about 9/3/14.
