29 CFR 1910.178(l)(1)(ii): Prior to permitting an employee to operate a powered industrial truck (except for training purposes), the employer shall ensure that each operator has successfully completed the training required by this paragraph (l), except as permitted by paragraph (l)(5).     a)      Holler Metal Fabricators, Inc., Metuchen, NJ site: Facility: Operators were not trained in the safe operation of powered industrial trucks such as, but not limited to Toyota 7FGCU25, etc. exposing them to a crushing hazard in the event of a rollover/tipover.        Condition noted on/or about September 5, 2014.           Training shall consist of a combination of formal instruction, practical training, and evaluation of the operators performance in the workplace.

Metuchen, NJ site: West Wall: Electrical panel boards were not maintained free of storage, in that, materials obstructed the access way.          a)      Electrical panel board rated 208 volts that controls the outlets employees use to power welding equipment was not maintained free of storage, in that, storage materials such as, file cabinets,  etc. obstructed the access way.      b)      Electrical panel board rated 480/277 volts that controls the outlets employees use to power welding equipment was not maintained free of storage, in that, welding equipment such as, but not limited to Linde Mode ATA300 obstructed the access way.           Condition noted on/or about September 5, 2014.
