29 CFR 1910.23(a)(2): Every ladderway floor opening or platform was not guarded by a standard railing with standard toeboard on all exposed sides (except at entrance to opening), with the passage through the railing either provided with a swinging gate or so offset that a person cannot walk directly into the opening: On or about September 12, 2014, ladder platforms on grain bins were not equipped with swing gates and were not offset from the ladderway opening. The list of grain bins that lacked ladder platform swing gates or offsets includes, but is not limited to, grain bins 1-4, 5, and 7.
