29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):  (a) Jobsite, Warrendale Pointe Apartments, Building 2, Warrendale, PA:  On or about 09/08/14 an employee engaged in residential construction activities was not protected from falling by the use of guardrails, safety nets or personal fall arrest systems exposing the employee to a fall hazard of approximately 22 feet.

29 CFR 1926.503(c)(3): Retraining was not conducted when inadequacies in an affected employee's knowledge or use of fall protection systems or equipment indicate that the employee has not retained the requisite understanding or skill:  (a) Jobsite, Warrendale Pointe Apartments, Building 2, Warrendale, PA:  On or about 09/08/14 retraining had not been conducted for an employee who was not utilizing fall protection and was exposed to 22 ft. fall hazards indicating that the employee had not retained the requisite understanding and skill needed to work safely.
