29 CFR 1926.502(b)(1): The top edge height of the top rails, or equivalent guardrail system members, was not 42 inches (1.1 m) plus or minus 3 inches (8 cm) above the walking/working level: a) Bloom Roofing Systems, Inc. on or about September 08, 2014, at the address of 1500 E. Alexis Rd. Toledo, OH: The employer failed to ensure employees were provided with a guardrail system that was 42 inches high (+/- 3 inches). Employees performing roofing activities atop a commercial structure with a 30 in. high parapet railing were exposed to an approximate 25 ft. fall to the ground below.
