Employees were exposed to amputation and crushing injuries while setting up CNC lathes and automatic screw machines such as, but not limited to, the #2, #3 and #9 Browne & Sharpe machines, and the Mori Seki CNC lathe.

29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:    a. McDaniel Products, Inc. dba Automatic Parts - Mansfield, OH: On or about September 5, 2014, employees operating the Tongtai (model TNL-120ALII) CNC machine were exposed to injuries from the machine's rotating parts and/or flying chips.  The machine was not adequately guarded in that employees were not prevented from accessing the hazard area during operation.    b. McDaniel Products, Inc. dba Automatic Parts - Mansfield, OH: On or about September 8, 2014, employees operating the Mori Seiki SL-4 CNC machine were exposed to injuries from the machine's rotating parts and/or flying chips.  The machine was not adequately guarded in that employees were not prevented from accessing the hazard area during operation.    c. McDaniel Products, Inc. dba Automatic Parts - Mansfield, OH: On or about September 8, 2014, employees operating the Cincom Citizen L-20 CNC lathe machine (s/n 00572) were exposed to injuries from the machine's rotating parts and/or flying chips.  The machine was not adequately guarded in that employees were not prevented from accessing the hazard area during operation.    d. McDaniel Products, Inc. dba Automatic Parts - Mansfield, OH: On or about September 8, 2014, the employer did not assure that the chuck of the Rockwell drill press #33 (s/n 1602444) was guarded to protect employees from hazards associated with the rotating part.    e. McDaniel Products, Inc. dba Automatic Parts - Mansfield, OH: On or about September 8, 2014, the employer did not assure that the chuck of the Snow DR-2 drill press (s/n M39853-674) was guarded to protect employees from hazards associated with the rotating parts.    f. McDaniel Products, Inc. dba Automatic Parts - Mansfield, OH: On or about September 8, 2014, the employer did not assure that the chuck of the Snow drill press (s/n 36358-7) was guarded to protect employees from hazards associated with the rotating part.    g. McDaniel Products, Inc. dba Automatic Parts - Mansfield, OH: On or about August 8, 2014, employees that operated the #2 Brown & Sharpe screw machine (s/n 542-00-7840) were exposed to injuries from the machine's rotating parts, pinch points, and flying chips.  The machine was not adequately guarded in that employees were not prevented from accessing the hazard area during operation.    h. McDaniel Products, Inc. dba Automatic Parts - Mansfield, OH: On or about September 8, 2014, employees that operated the #9 Brown & Sharpe screw machine (s/n 542-2-7512-1) were exposed to injuries from the machine's rotating parts, pinch points, and flying chips.  The machine was not adequately guarded in that employees were not prevented from accessing the hazard area during operation.    i. McDaniel Products, Inc. dba Automatic Parts - Mansfield, OH: On or about September 8, 2014, employees that operated the #11 Brown & Sharpe screw machine (s/n 542-2-7806-1) were exposed to injuries from the machine's rotating parts, pinch points, and flying chips.  The machine was not adequately guarded in that employees were not prevented from accessing the hazard area during operation.
