29 CFR 1926.501(b)(4)(i): Each employee on walking/working surfaces was not protected from falling through holes (including skylights), more than six feet (1.8 m) above lower levels, by personal fall arrest systems, covers, or guardrail systems erected around such holes:    a) Architectural Artifacts, Second Level - On or about September 3, 2014, the employer failed to ensure employees were provided with personal fall arrest systems, covers, or guardrail systems while working around floor holes, such as the existing floor hole on the second story of the building.  Workers were exposed to a fall hazard of approximately thirteen feet, five inches, while removing a railing system and not being provided with any other means of fall protection.     b) Architectural Artifacts, Second Level - On September 11, 2014, the employer failed to ensure employees were provided with personal fall arrest systems, covers, or guardrail systems while working around floor holes, such as the existing floor hole on the second story of the building.  Workers were exposed to a fall hazard of approximately thirteen feet, five inches, while removing floor joists.      In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification.  This documentation may include, but is not limited to, evidence of the purchase or repair of the equipment, photographic or video evidence of abatement, or other written records.
