29 CFR 1910.26(c)(2)(iv): Ladder(s) were not maintained in a good usable condition at all times. a.) Meijer Stores Limited Partnership located at 7240 W. Central Ave. Toledo, OH; On or about September 9, 2014, the employer did not ensure the Rol-Away 3 Step ladder carts were maintained in a good usable condition. Per ISA this sentence removed: On September 4, 2014 an employee was conditioning the grocery shelves and fell to the ground while stepping down off of the ladder cart, falling on his back and head, resulting in a head laceration and back fractures.

Central Ave. Toledo, OH; On or about September 9, 2014, the employer did not assure the Rol-Away 3 step ladder carts with defects were taken out of service. Per ISA this sentence removed:On September 4, 2014 an employee fell off the second step of a Rol-Away cart and was injured in that ladder did not have an operational brake mechanism and was not equipped with rubber stoppers.
