29 CFR 1926.501(b)(10): Each employee engaged in roofing activities on low-slope roofs with unprotected sides and edges 6 feet or more above lower levels, was not protected from falling by guardrail systems, safety net systems, personal fall arrest systems, or a combination of warning line system and guardrail system, warning line system and safety net system, or warning line system and personal fall arrest system, or warning line system and safety monitoring system.  Or, on roofs 50-feet (15.25 m) or less in width, each employee was not protected by use of a monitoring system.      a) D & D Roofing, Inc. at 14694 Orchard Parkway, Westminster, CO 80030:  On or before September 9th, 2014, employees were exposed to a fall hazard while conducting roofing activities on a low-slope roof.  All employees were engaged in work and the employer failed to ensure that employees were protected through the use of fall protection.      The employer, D & D Roofing, was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(b)(10), which was contained in OSHA inspection number 98958, citation number 1, item number 1, and was affirmed as a final order on 10-12-2011, with respect to a workplace located at 3545 Larimer St., Denver, CO 80205.
