Employees were performing roof work including shingle removal, tar paper placement, and shingle placement.

29 CFR 1926.501(b)(11): Each employee on a steep roof with unprotected sides and edges 6 feet (1.8 m) or more above lower levels was not protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems: On or about August 14, 2014, employees were performing roofing work on a residential home at 30972 Schwarz Road, Westlake, OH, without the use or utilization of fall protection devices. The roof was measured as having slopes of both 6/12 and 12/12. The fall to the level below was approximately 20 feet.
