29 CFR 1926.100(a): Employees working in areas where there is a possible danger of head injury from impact, or from falling or flying objects, or from electrical shock and burns, shall be protected by protective helmets.  On or about September 9, 2014, employees working on the ground level and below other workers were not protected from falling or flying objects. Employees were observed framing and securing decking on the second level while employees worked both at ground level and on the first level. The hazard was located at 1n Cunningham Road, Salem, OH

29 CFR 1926.102(a)(1): Eye and face protective equipment were not used when machines or operations presented potential eye or face injury from physical, chemical, or radiation agents.  On or about September 9, 2014, employees were not provided or utilizing eye protection while using pneumatic air nail guns for securing wood decking. The hazard was observed onsite at 1n Cunningham Road, Salem, OH.

29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems.  a) On or about September 9, 2014, employees were working atop of the first floor decking without the use of fall protection devices. An employee was saw cutting lumber for use in framing while standing on the elevated work surface located at 1n Cunningham Road, Salem, OH. The fall height measured 12 feet  b) On or about September 9, 2014, employees were working atop of the second floor decking without the use of fall protection devices. Employees were installing floor decking while standing on the elevated work surface located at 1n Cunningham Road, Salem, OH. The fall height was measured as approximately 23 feet.

29 CFR 1926.501(b)(4)(ii): Each employee on a walking/working surface shall be protected from tripping in or stepping into or through holes (including skylights) by covers.  On or about September 9, 2014, floor holes were not covered to protect employees from falling or tripping. The floor hole was located directly next to and below a step ladder which was used for access to the level above. This was onsite at a worksite located at 1n Cunningham Road, Salem, OH.
