The ladder had not been inspected by a competent person and removed from use. The ladder had the following defects:  A. Bent ladder rungs; employees were exposed to falls from the ladder.  B. Half of an extension ladder was being utilized which did not have protective caps on the top ends of the side rails; employees were exposed to lacerations.
