29 CFR 1926.651(c)(2): Means of egress from trench excavations. A stairway, ladder, ramp or other safe means of egress was not located in trench excavations that are 4 feet (1.22 m) or more in depth so as to require no more than 25 feet (7.62 m) of lateral travel for employees.   At this excavation site, employees entered a vertical wall trench which was over 5 feet deep to explore for pipe. There was not a ladder in the trench for easy access or egress from trench.

29 CFR 1926.652(a)(1): Each employee in an excavation shall be protected from cave-ins by an adequate protective system designed in accordance with paragraph (b) or (c) of this section:   At this excavation site, employees worked inside an excavation greater than 5 feet deep without an employee protection system. Employees were exposed to hazards associated with cave-ins.

29 CFR 1926.1053(b)(15): Ladders were not inspected by a competent person for visible defects on a periodic basis and after an occurrence that could affect their safe use  At this job site, employees used a available portable, metal ladder for ingress and egress from a trench. The ladder had not been inspected by a competent person and removed from use. The ladder had the following defects:  A. Bent ladder rungs; employees were exposed to falls from the ladder.  B. Half of an extension ladder was being utilized which did not have protective caps on the top ends of the side rails; employees were exposed to lacerations.
