LOCATION: 268 Bedford Ave, Brooklyn, NY 11249, 5th floor elevator shaft.    A). Employees using a platform in the elevator shaft of the 5th floor to gain access to the roof level were not protected from falling through floor holes or openings by use of guardrails, covers, safety nets, or personal fall arrest systems.
