CFR 1926.1412(c)(1): Upon completion of assembly, the equipment was not inspected by a qualified person to assure that it was configured in accordance with the manufacturer equipment criteria: a) 1 Robert Wood Johnson Place, New Brunswick, NJ 08901. Upon completion, the employers qualified person did not inspect the equipment to assure that it was configured in accordance with the manufacturers equipment criteria. 1) Employees were exposed to crushing and struck-by hazards when the auxiliary line failed during a critical pick resulting in the auxiliary hook and load falling to the ground. Incident occurred on or about 07/29/14.
