29 CFR 1910.1030(c)(1)(v): The employer, who is required to establish an Exposure Control Plan, did not solicit input from non-managerial employees responsible for direct patient care who are potentially exposed to injuries from contaminated sharps in the identification, evaluation and selection of effective engineering and work practice controls and did not document the solicitation in the Exposure Control plan:  a) The employer did not solicit input from non-managerial employees responsible for direct patient care in the identification, evaluation and selection of needle devices used at the facility. The injury was recorded on the log as occurring on March 3, 2012. Observed on or about September 3, 2014.    b) The employer had entered the employee's name for a needlestick injury on the OSHA 300 log for calendar year 2012.  The injury was recorded on the log as occurring on November 16, 2012. Observed on or about September 3, 2014.    c) The employer had entered the employee's name for a needlestick injury on the OSHA 300 log for calendar year 2013. The injury was recorded on the log as occurring on March 13, 2013. Observed on or about September 3, 2014.    d) The employer had entered the employee's name for a needlestick injury on the OSHA 300 log for calendar year 2013. The injury was recorded on the log as occurring on June 16, 2013. Observed on or about September 3, 2014.    e) The employer had entered the employee's name for a needlestick injury on the OSHA 300 log for calendar year 2013. The injury was recorded on the log as occurring on September 27, 2013. Observed on or about September 3, 2014.    f) The employer had entered the employee's name for a needlestick injury on the OSHA 300 log for calendar year 2014. The injury was recorded on the log as occurring on January 6, 2014. Observed on or about October 7, 2014.
