29 CFR 1910.132(a): Protective equipment was not used when necessary whenever hazards capable of causing injury and impairment were encountered:         a.) On or about September 10, 2014, the employer did not ensure that employees working in the pouring area wore personal protective equipment such as but not limited to, hard hat, face shield, apron, leggings, fire resistant clothing (foundry greens) to protect from molten metal.

29 CFR 1910.1000(c): An employee's exposure to respirable silica listed in Table Z-3 exceeded the 8-hour time weighted average limit calculated for that substance in the table:      a.) On or about September 10, 2014, an employee working in the shake out area was overexposed to silica at an 8 hour time weighted average of 4.013 mg/m3, which exceeded the calculated permissible exposure limit of 3.030 mg/m3 by 132%.

On or about September 10, 2014, the employer failed to implement engineering and/or administrative controls to control exposures to respirable silica.  An employee working in the shake out area was overexposed to silica at an 8 hour time weighted average of 4.013 mg/m3, which exceeded the calculated permissible exposure limit of 3.030 mg/m3 by 132%.

On or about September 10, 2014 The employer did not develop and implement a written respiratory protection program to protect employees from work place respiratory hazards.  An employee exceeded the calculated permissible exposure limit of respirable silica in the shake out area of the facility.
