29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems. This violation occurred on or about September 10, 2014, and at times prior thereto, when employees were exposed to falls from heights: a. On the north-west side of the building, an employee was observed working at the edge of a roof approximately 10 feet above a concrete surface without the use of guardrails or fall protection; b. Two employees were working on a roof approximately 40 feet above a concrete surface without the use of guardrails or fall protection.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladders length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed: This violation occurred on or about September 10, 2014, and at times prior thereto, when employees were using an extension ladder to access a roof approximate 10 feet off of the ground without the ladder side rails extending at least 3 feet above the upper landing surface being accessed.
