Site - On or about September 11, 2014, employees who were assigned to operate the CTM (model SHG18N8T S/N A16N10497) propane forklift were not provided training for its use. Achilles of Milford, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1910. 178(l)(1)(ii), which was contained in OSHA inspection number 953739, citation number 1, item number 3, and was affected as a final order date on 4/12/14, with respect to the workplace located at the 51 Elm St., Milford, NH 03055.
