29 CFR 1926.20(b)(2): The employer did not initiate and maintain programs which provided for frequent and regular inspections of the job site, materials and equipment to be made by a competent person(s): On or about September 9, 2014, during construction activities at a worksite located at NW Corner of State and West Bath Roads, Cuyahoga Falls, OH, the employer did not ensure frequent and regular inspections were conducted at the worksite where subcontractor employees were with fall hazards approximately 25-30 feet and in aerial lifts with no fall protection.
