29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:     On or about September 8, 2014, and at times prior thereto, employees were allowed to operate machines that were found not having guards provided, including but not limited to the following instances:     1) At the beef grinder area in 54th building, an auger was provided with cover but did not have an interlock device provided.     2) At the bailer area in Enterprise building, a bailer being operated without an interlock device provided.     3) At the final meat area in Enterprise building, a meat grinder being operated without a cover provided.     4) In the maintenance shop area in Enterprise building, a manual lathe was found not having a chuck guard provided.     Employees were exposed to possible amputation and crushing hazards.

29 CFR 1910.215(b)(9): Guard for abrasive wheel machine where the operator stands in front of the machine was not constructed so that the peripheral protecting member could be adjusted to the constantly decreasing diameter of the wheel:    On or about September 8, 2014, and at times prior thereto, in the maintenance shop area in 54th building the employer allowed employees operating abrasive wheel grinding machines which did not have tongue guard provided.  Employees were exposed to flying fragments in the event of an exploding wheel.

29 CFR 1910.215(d)(3): The contact surface(s) of wheel(s), blotter(s) or flange(s) on grinding machine(s) were not flat and free or foreign matter:    On or about September 8, 2014, and at times prior thereto, in the maintenance shop area in 54th building the employer allowed employees to operate abrasive wheel grinding machines that had the abrasive wheels worn unevenly.  The employees were exposed to flying fragments in the event of exploding wheels.
