29 CFR 1910.106(b)(6): In locations where flammable vapors may be present, precautions were not taken to prevent ignition by eliminating or controlling sources of ignition. Sources of ingition may include open flames, lightning, smoking, hot surfaces, frictional heat, sparks (static, electrical, and mechanical), spontaneous ignition, chemical and physical-chemical reactions, and radiant heat: At the Karlin lease, northwest of Catherine, KS, an employee was not protected from the hazards of hydrogen sulfide in that the employee drained fluids from a crude oil storage tank (gun barrel) equipped with a heating element, which caused the gas to ignite and explode, exposing the employee to burns.
