29 CFR 1910.119(j)(4)(i): The employer did not perform inspection and tests on process equipment. An independent inspection was not conducted at least every five years in accordance with the recommended and generally accepted good engineering practices such as, but not limited to, IIAR Bulletin No. 109, A Guide to Good Practices for the Operation of an Ammonia Refrigeration System, on the following pressure vessels such as, but not limited to, the Frigo: Accumulator, Intercooler, Liquid Transfer Vessel, High Pressure Receiver (Thermosyphon), Evaporator Condenser, and Gas Purger, along with the House: High Pressure Receiver, High Temperature Accumulator, Low Temperature Accumulator, and Evaporator Condenser. There is no abatement certification or documentation required for this item.

29 CFR 1910.119(f)(1)(iii)(B): The employer failed to ensure that the Process Safety Management Standard Operating Procedures included safety and health consideration precautions necessary to prevent exposure, including engineering controls, administrative controls, and personal protective equipment. The employer did not ensure that the Frigo and House written Standard Operating Procedures (SOP) for oil pot draining included the requirements of wearing respiratory protection, as a necessary precaution, to prevent respiratory irritation caused by ammonia exposure. Abatement certification is required of this item in accordance with the requirements of 29 CFR 1903.19(c).
