An employee of a different employer leaned against the mid rail of the second floor guardrail system that was installed by Onsite Safety Inc. and fell approximately 12 feet to the ground below when the guard rail anchor pulled out of the wall.

The mid rail and top rail anchors of the cable system were initially installed to close to the outer edge of the column and had to be reinstalled in the center of the column. One of the anchors that was reinstalled failed causing an employee of a separate company to fall.
