29 CFR 1926.502(b)(5): Midrails, screens, mesh, intermediate vertical members, solid panels, and equivalent structural members were not capable of withstanding, without failure, a force of at least 150 pounds (666 N) applied in any downward or outward direction: a) For employees working at 5128 Tidewater Preserve Blvd in Bradenton, as observed on or about July 10, 2014. An employee of a different employer leaned against the mid rail of the second floor guardrail system that was installed by Onsite Safety Inc. and fell approximately 12 feet to the ground below when the guard rail anchor pulled out of the wall.

29 CFR 1926.503(c): When the employer has reason to believe that any affected employee who has already been trained does not have the understanding and skill required by paragraph (a) of this section, the employer is to retrain each such employee such as inadequacies in an affected employees knowledge or use of fall protection systems or equipment indicate that the employee has not retained the requisite understanding of skill. a) For employees working at 5128 Tidewater Preserve Blvd in Bradenton, as observed on or about July 10, 2014. The mid rail and top rail anchors of the cable system were initially installed to close to the outer edge of the column and had to be reinstalled in the center of the column. One of the anchors that was reinstalled failed causing an employee of a separate company to fall.
