29 CFR 1926.1053(b)(16): Portable ladder(s) with structural defects, such as, but not limited to, broken or missing rungs, cleats, or steps, broken or split rails, corroded components, or other faulty or defective components, were not either immediately: a. On or about September 10, 2014, employees of R.A. Scott Construction used a Werner 14-foot extension ladder to access and egress from the trench. The ladder had damage such as a large split on the rails, exposing the employees to fall hazards. b. On or about September 10, 2014, employees of T & R Tapping Service, Inc. were using a Werner 14-foot extension ladder in the trench. The ladder had damage such as a large spilt on the rails and was placed in the trench upside down with the feet sticking upward out of the trench, exposing the employees to fall hazards. Abatement Certification and Documentation are required
