29 CFR 1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section: (a)Electronic Recyclers International Inc. at 3250 Abilene Street, Aurora, CO: On and before September 9th, 2014, Electronic Recyclers International Inc did not ensure that procedures were developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section. A contractor serviced the JV Manufacturing Inc model HE-60 horizontal baler. An authorized employee of Electronic Recyclers International Inc shut off the electrical power to the baler. The horizontal baler was provided with two hazardous energy sources: electrical and hydraulic. The horizontal baler did not have a procedure for the control of the electrical and hydraulic energies. This condition exposed the contractor and the employee of Electronic Recyclers International Inc to uncontrolled energy hazards.

29 CFR 1910.151(c): Where employees were exposed to injurious corrosive materials, suitable facilities for quick drenching or flushing of the eyes and body were not provided within the work area for immediate emergency use: (a)Electronic Recyclers International Inc, at 3250 Abilene Street, Aurora, CO: On and before September 25th, 214, Electronic Recyclers International Inc did not ensure that suitable facilities for quick drenching or flushing of the eyes and body were provided within the work area for immediate emergency use where employees were exposed to injurious corrosive materials. One employee performed maintenance activities, including adding water to a lead acid battery powered Hyster model E45XN-27 forklift. Lead acid batteries contain corrosive liquid sulfuric acid. The nearest emergency shower and eyewash was approximately thirty-five feet from the point of operation and was accessed by stepping over two knee-height barricades and walking around approximately four foot tall cardboard boxes containing electronics. This condition exposed the employee to a chemical burn hazard.

Employees disassembled electronics, including cathode ray tube televisions and LCD televisions. Wipe sampling of the work stations found accumulations of lead. This condition exposed employees to a lead hazard.

29 CFR 1910.1200(f)(5)(i): The employer did not ensure that each container of hazardous chemicals in the workplace was labeled, tagged or marked with the identity of the chemical contained therein: (a)Electronic Recyclers International Inc. at 3250 Abilene Street, Aurora, CO: On and before September 25th, 2014, Electronic Recyclers International Inc. did not ensure that each container of hazardous chemicals in the workplace was labeled, tagged or marked with the identity of the chemical contained therein. Employees disassembled electronics, including LCD televisions. LCD televisions contained lamps containing mercury. Lamps were removed from LCD televisions. Broken lamps were placed into a metal drum labeled "accidentally broken", while unbroken lamps were placed into a cardboard container. Neither the drum nor the cardboard container were labeled identifying that mercury was contained within the drum or cardboard box. Mercury vapor was detected in both containers. This condition exposed employees to a mercury hazard.
