29 CFR 1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section: (a) Nestle Waters at 11700 E. 147th Ave. Denver, CO 80239: On and before 9/5/14, the employer did not ensure employees utilized the written Lockout/Tagout (LOTO) procedures developed by the company for the control of hazardous energy. Specifically, the employer did not ensure that LOTO procedures are followed during maintenance on the Sidel Blowmolder. This equipment is powered by multiple energy sources to include electrical and pneumatic. This condition exposed employees to hazards associated with the sudden release of stored energy. Abatement Note: Abatement certification is required for this item (see enclosed "Certification of Corrective Action Worksheet").

29 CFR 1910.147(c)(6)(i): The employer shall conduct a periodic inspection of the energy control procedure at least annually to ensure that the procedure and the requirements of this standard are being followed. (a) Nestle Waters at 11700 E. 147th Ave. Denver, CO 80239: On and before 9/5/14, the employer did not conduct a periodic inspection of the energy control procedures for the control of hazardous energy. Documents provided by the company indicate that the Lockout/Tagout (LOTO) Safety Policy and Guidelines have not been updated since October of 2011 and the specific written procedures for equipment such as but not limited to; the Sidel Blowmolder have not been reviewed since 2007. This condition exposed employees to hazards associated with the sudden release of stored energy. Abatement Note: Abatement certification is required for this item (see enclosed "Certification of Corrective Action Worksheet").

(a) Nestle Waters at 11700 E. 147th Ave. Denver, CO 80239: On and before 9/5/14, the employer did not ensure that unused openings in the electrical panels were effectively closed. Numerous electrical panels had missing breakers which had not been closed. This condition exposed employees to the hazard of electrical shock.
