29 CFR 1910.101(b): Section 3.1.11 Compressed Gas Association Pamphlet P-1-1965, as adopted by 29 CFR 19101(b):  before returning empty cylinders, the valve protective cap was not replaced:    a)  Booster room, Dock 4    A used 120 pound cylinder of chlorine gas being returned to the supplier fell and started leaking while being removed from a gas cabinet without the valve protective cap in place.  Chlorine concentrations determined by direct reading instruments and in place monitors indicated concentrations at or above 10 parts per million, the level Immediately Dangerous to Life and Health (IDLH).      Violation occurred on or about 9/13/14.
