On or about September 15th, 2014, at the above addressed jobsite, an employee was exposed to amputation/laceration hazard while operating a Tractor, towing a Spray Rig, with an unguarded PTO drive (Drive shaft).

On or about September 15th, 2014 at the above addressed site, an employee was exposed to amputation/laceration hazards while operating a Spray Rig with an unguarded chain and sprocket.
