29 CFR 1904.33(a): The employer did not save the OSHA 300 Log, the privacy case list (if one existed), the annual summary, and the OSHA 301 Incident Report Forms for five (5) years following the end of the calendar year that these records covered. a) Located at Reed Perrine, Tennet, NJ, USA: On or about 9/15/14, the employer did not develop and maintain the 2012 Injury/Illness Log (OSHA Form 300 or its equivalent) or the Annual Summary (OSHA Form 300A or the equivalent) for at least five years. b) Located at Reed Perrine, Tennet, NJ, USA: On or about 9/15/14, the employer did not develop and maintain the 2013 Injury/Illness Log (OSHA Form 300 or its equivalent) or the Annual Summary (OSHA Form 300A or the equivalent) for at least five years. NOTE: IN ADDITION TO ABATEMENT CERTIFICATION, THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT DOCUMENTATION FOR THIS ITEM, FAILURE TO COMPLY WILL RESULT IN AN ADDITIONAL PENALTY OF $1000.00 IN ACCORDANCE WITH 29 CFR 1903.19.
