29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems:    (a) Jobsite, Frick Building, Pittsburgh, PA:  On or about 09/17/14 an employee working on an unprotected edge approximately 16 feet above the ground was not protected from falling by guardrail systems, safety nets or a personal fall arrest system.

29 CFR 1926.503(c)(3): Retraining was not conducted when inadequacies in an affected employee's knowledge or use of fall protection systems or equipment indicate that the employee has not retained the requisite understanding or skill:  (a) Jobsite, Frick Building, Pittsburgh, PA:  On or about 09/17/14 retraining had not been conducted when an employee was observed working at heights up to 16 feet without fall protection systems in place indicating the employee had not retained the understanding or skill needed to protect them from potential fall hazards.
