29 CFR 1910.22(a)(2): Floor(s) of workroom(s) were not maintained in, as far as possible, dry condition: a) Sheeting Lines- The employer did not ensure sure that the floor areas, such as on the west side of the Sheeting Line #2 Proofer, were kept in as dry as possible of a condition to prevent slips and falls. Water from the malfunctioning Proofer was leaking out of the machine on a continual basis while it was in operation and the water flowed over the walking path of the employees that operating Sheeting Line #1 and #2. b) Rack Washing Area- The employer did not ensure sure that the floor areas, such as the area in front of the East Rack Washer, were kept in as dry as possible of a condition to prevent slips and falls. The bottom seal on the East Rack Washer was deteriorated and it allowed water to flow freely out of the washer when it was in operation. In addition the ramp plate was left in place while the machine ran which allowed the water to flow over the floor area where employees had to walk. 29 CFR 1910.303(g)(2)(i): Except as elsewhere required or permitted by Subpart S of Part 1910, live parts of electric equipment operating at 50 volts or more were not guarded against accidental contact by use of approved cabinets or other forms of approved enclosures or by any of the means identified in paragraphs (A), (B), (C), and (D) of 29 CFR 1910.303(g)(2)(i): a) Receiving Dock- The employer failed to ensure live parts, such as inside the Panel Box L1 on the Receiving Dock, were guarded against accidental contact by use of approved cabinets. The doors of many of the panel boxes were found open including Panel Box L1, which had a missing circuit breaker and that exposed the energized bus bar. The door to the box would need to be in the closed position to be a form of an approved cabinet or enclosure. 29 CFR 1910.305(e)(1): Cabinets, cutout boxes, fittings, boxes, and panelboard enclosures in wet locations were not installed to prevent moisture or water from entering and accumulating. The enclosures were not weatherproof in the wet locations: a) Rack Washing Area- The employer failed to ensure all boxes and fittings in damp or wet locations, such as the outlet next to the Rack Washer Sink, were installed to prevent moisture and water from entering and accumulating. This box was originally installed to be weatherproof, but the cover had been broken off and it was no longer maintained in a weatherproof condition to prevent water from entering and accumulating. In addition, this outlet was not equipped with ground fault circuit interruption protection. b) Production Area- The employer failed to ensure all fittings in damp or wet locations, such as the two drop down outlet plugs on the wall above the sink in the Icing Area, were installed to prevent moisture and water from entering and accumulating. These plugs were not weatherproof, showed signs of damage and corrosion, and were not equipped with ground fault circuit interruption protection.
