29 CFR 1926.102(a)(1): Eye and face protective equipment were not used when machines or operations presented potential eye or face injury from physical, chemical, or radiation agents:   On or about September 17, 2014, at the above addressed site, an employee was exposed to potential struck by eye injury when using a Dewalt hammer drill to install a new sign on a masonry wall while not wearing eye protection equipment.

29 CFR 1926.453(b)(2)(v): A body belt was not worn and/or a lanyard was not attached to the boom or basket when working from an aerial lift:   On or about September 17, 2014, at the above addressed site, an employee was exposed to a potential fall hazard of approximately 13 feet 6 inches above a lower level when installing a sign from the basket of an aerial lift while not wearing a harness with a lanyard attached to the basket or boom of that lift.

29 CFR 1926.454(a): The employer did not have each employee who performed work while on a scaffold trained by a person qualified in the subject matter to recognize the hazards associated with the type of scaffold being used and to understand the procedures to control or minimize those hazards:  On or about September 17, 2014, at the above addressed site, an employee exposed to a fall hazard while operating an aerial lift was not provided with a training program that would enable him to recognize that hazards associated with falls while using that lift.   PLEASE NOTE: CERTIFICATION OF ABATEMENT IS REQUIRED FOR THIS VIOLATION.
