On or about September 17, 2014 an employee was observed welding from an unprotected side at a height of more than 6 feet without the use of fall protection. The employee was exposed to a fall hazard of approximately 19 feet.

On or about September 17, 2014 an employee was observed welding from an unprotected side at a height of more than 6 feet without the use of fall protection.
