29 CFR 1910.333(c)(3): When work was to be performed near overhead lines, the lines were not de-energized and grounded, nor were other protective measures provided before work was started:   a) Back yard of residential home- an employee was exposed to energized overhead power lines in that he was trimming a palm frond and was approximately 44 inches from a 13.2 kv power line, on or about 6/30/14.

29 CFR 1910.333(c)(3)(i)(A)(1): When an unqualified person was working in an elevated position near overhead lines with voltages to ground rated at 50 kV or below, the location was not such that the person and the longest conductive object he or she may contact cannot come closer to any unguarded, energized overhead line than the following distance of 10 feet (305 cm).   a) Back yard of residential home- an employee was exposed to energized overhead power lines in that he was trimming a palm frond and was approximately 44 inches from a 13.2 kv power line, on or about 6/30/14.

29 CFR 1904.39(a): The employer did not orally report the death of an employee or the in-patient hospitalization of three or more employees because of a work-related incident  a) On or about 7/5/14, the employer failed to report to OSHA a work-related death which occurred on 7/5/14. The accident was not reported to OSHA until 9/10/14.
