29 CFR 1926.451(g)(1)(vii): For all scaffolds not otherwise specified in paragraphs (g)(1)(i) through (g)(1)(vi) of this section, each employee was not protected by the use of personal fall arrest systems or guardrail systems meeting the requirements of paragraph (g)(4) of this section.  a) Jobsite: Pump Jack Scaffold - Employees working from a pump jack scaffold were not protected from falling by the use of a personal fall arrest system or guardrail system.  Employees were performing siding activities approximately 13 feet above ground level, on or about 9/18/14.

29 CFR 1926.454(a): The employer did not have each employee who performed work on a scaffold be trained by a person qualified in the subject matter to recognize the hazards associated with the type of scaffold being used, and to understand the procedures to control or minimize those hazards.   a) Jobsite: Pump Jack Scaffold- The employer did not train their employees to recognize the hazards that are present when working from pump jack scaffolds, and to understand the procedures to control or minimize those hazards, on or about 9/18/14.

29 CFR 1926.501(b)(10): Each employee engaged in roofing activities on low-slope roofs, with unprotected sides and edges 6 feet or more above lower levels was not protected from falling by the use of a guardrail system, safety net system, personal fall arrest system, warning line system, or a safety monitoring system.  a) Jobsite: Roof - Employees performing roofing activities on a low slope roof and in close proximity to unprotected sides and edges, were exposed to fall hazards of approximately 25 feet without the use of a fall protection system, on or about 9/18/14.
