Listed, labeled, or certified equipment shall be installed and used in accordance with instructions included in the listing, labeling, or certification: a. On or about September 16, 2014, at the above addressed jobsite, employees operated a Worm Drive Saw (Skilsaw- HD77) that had a splice which did not conform to the original manufacture's listing and certification, thereby exposing employees to electrical hazards.

On or about September 16, 2014, at the above addressed jobsite, the extension cord used to operate power tools, such as but not limited to a Worm Drive Saw, was frayed, thereby exposing employees to electrical hazards. b. On or about September 16, 2014, at the above addressed jobsite, employees operated a Worm Drive Saw (Skilsaw- HD77) that had a frayed electrical cord, thereby exposing employees to electrical hazards.

29 CFR 1926.501(b)(13): "Residential construction." Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels shall be protected by guardrail systems, safety net system, or personal fall arrest system unless another provision in paragraph (b) of this section provides for an alternative fall protection measure. a. On or about September 16, 2014, at the above addressed jobsite, employees performing roofing operations, approximately twenty-five (25) feet above the ground were not protected from falls, thereby exposing employees to fall hazards.
