29 CFR 1910.252(a)(2)(vi)(C): Cutting or welding was permitted in the presence of explosive atmospheres (mixture of flammable gases, vapors, liquids, or dusts with air), or explosive atmospheres that could develop inside uncleaned or improperly prepared tanks or equipment which had previously contained such materials, or that could develop in areas with an accumulation of: (a) Haines Engineering, Inc. 6262 South 550 East, Berne, Indiana: On or about September 18, 2014, at the work site located at 417 W. Market Street, Celina, Ohio, the employer did not ensure that the grain storage tank that was under repair did not contain an explosive atmosphere. Employees were at risk of serious injury from fire and deflagration hazards while welding on bin #7 which contained an explosive atmosphere of gain dust. (b) Haines Engineering, Inc. 6262 South 550 East, Berne, Indiana: On or about September 18, 2014, at the work site located at 417 W. Market Street, Celina, Ohio, the employer did not ensure that the area of the mill building beneath bin #7, where welding repairs occurred, did not contain an accumulation of combustible dust that developed into an explosive atmosphere.

Market Street, Celina, Ohio, the employer did not secure authorization from the lead mill operator to perform welding on bin #7 located in the mill building. Employees were at risk of serious injury from fire and deflagration hazards while welding on bin #7 which contained an explosive atmosphere of gain dust.
