29 CFR 1926.451(g)(1)(i): Each employee on a boatswains' chair, catenary scaffold, float scaffold, needle beam scaffold, or ladder jack scaffold was not protected by a personal fall arrest system:    a) Danny Bronikowski dba Next Generation Roofing, Toledo, Ohio:  On or about September 16, 2014, in the Woodmont Subdivision, Lot #8, 1 Basswood Dr., Perrysburg, Ohio, the employer did not ensure that employees were protected from falls while working from a ladder jack scaffold.  Employees were exposed to falls of approximately 19 feet to the ground below during roofing activities on a residential type structure.  No form of fall protection was in place at the time of the inspection.
