29 CFR 1910.23(a)(8): Every floor hole into which persons can accidentally walk were not guarded.    The employer does not guard holes into which persons can accidently walk.  This violation was observed on or about September 15, 2014 at the waste water treatment plant where an employee was exposed to a fall hazard when holes in the floor were not guarded.       Pursuant to 29 C.F.R. 1903.19, within ten (10) calendar days of the abatement date, the employer must submit documentation showing that it is in compliance with the standard, including describing the steps that it is taking to ensure every floor hole into which persons can accidentally walk are guarded.

29 CFR 1910.23(c)(1): Open-sided floors and/or platforms four feet or more above adjacent floor or ground level were not guarded with standard railings (or equivalent).      The employer does not guard open-sided floors and/or platforms four feet or more above adjacent floor or ground level with standard railings (or equivalent) exposing employees to injury. This violation was observed on or about September 15, 2014 on the waste water treatment plant tanks where an employee was exposed to a fall hazard when working on the aeration basin and surge tanks.       Pursuant to 29 C.F.R. 1903.19, within ten (10) calendar days of the abatement date, the employer must submit documentation showing that it is in compliance with the standard, including describing the steps that it is taking to ensure that all open-sided floors and/or platforms four feet or more above adjacent floor or ground level are guarded.

29 CFR 1910.333(c)(3)(i)(A): When an unqualified person is working in an elevated position near overhead lines, the location shall be such that the person and the longest conductive object he or she may contact cannot come closer to any unguarded, energized overhead than 10 feet for voltages to ground 50kV or below.    The employer did not insure that when an unqualified person is working in an elevated position near overhead lines, the location shall be such that the person and the longest conductive object he or she may contact cannot come closer to any unguarded, energized overhead line than 10 feet for voltages to ground 50kV or below.  This violation was observed on or about September 15, 2014 on the clarifier section of the waste water treatment plant tanks where an employee was exposed to a an electrical hazard.     Pursuant to 29 C.F.R. 1903.19, within ten (10) calendar days of the abatement date, the employer must submit documentation showing that it is in compliance with the standard, including describing the steps that it is taking to ensure the overhead lines are at least 10 feet away from longest conductive object the employee may use to contact the energized overhead line.
