29 CFR 1926.451(a)(6): Scaffolds were not designed by a qualified person and were not constructed and loaded in accordance with that design:    a) The employer did not ensure that the scaffold used by the employees, was designed, constructed and loaded to allow standing on the rails.  Employees were exposed to fall hazards from a height of up to 18 feet to a concrete surface while stepping on the mid- and top- rails of a Skyjack SJ6826RT Scissors Lift in order to gain access to the roof of the building.  The scissors lift manufacturer prohibits standing or climbing on the guardrails.     No abatement certification or documentation is required for this item.
