29 CFR 1926.651(b)(3): The exact location of the installation was not determined by safe and acceptable means when milling operations approached the location of underground installations, i.e. surface mounted gas valve box: a) Intersection of Spotswood Gravel Hill Road and Schoolhouse Road, Monroe Township, NJ 08831. The employer allowed his employees to approach the estimated location of underground installations, i.e. surface mounted gas valve box while milling the road surface when the exact location of the installation was not determined by safe and acceptable means resulting in hitting the gas valve box and rupturing the gas line. Incident occurred on or about 08/28/14.
