29 CFR 1926.503(a)(2)(iii): The employer did not assure that each employee had been trained, as necessary, by a competent person qualified in the use and operation of guardrail systems, personal fall arrest systems, safety net systems, warning line systems, safety monitoring systems, controlled access zones, and other protection to be used: a) On September 19, 2014 employees did not recognize or acknowledge the hazard of performing roofing activities on a 10 in 12 pitched roof without a fall protection system in place. Appropriate actions to minimize the hazard were not taken. b) On September 19, 2014 an employee did not recognize or acknowledge the hazard of using an incomplete fall protection system. The employee attached a snaphook and lanyard his tool belt for fall protection. Note) Abatement certification IS required for this item.
