29 CFR 1926.451(g)(1)(i): Each employee on a boatswains' chair, catenary scaffold, float scaffold, needle beam scaffold, or ladder jack scaffold shall be protected by a personal fall arrest system: a) On or about September 17, 2014 - at the jobsite located as 4611 S. Archer Avenue, Chicago, IL. - an employees was performing window installation from a ladderjack scaffold without the use of a personal fall arrest/protection system, thereby exposing the employee to fall hazards of approximately eighteen (18) feet.
