29 CFR 1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section: a. On or about Sept. 18, 2014, in the shipping area, an employee reached into a Turning Rolls Printer Station - New Roll Handling System (Equipment # 18-10610) to repair a broken bolt and was not protected from accidental start up due to not utilizing the documented lockout/tagout procedure, exposing the employees to a crushing, caught-in, and amputation hazard by the powered rollers and ejector bar.
