29 CFR 1910.138(a): The employer did not select and require employee(s) to use appropriate hand protection when employees' hands were exposed to hazards such as those from skin absorption of harmful substances; severe cuts or lacerations; severe abrasion; punctures; chemical burns; thermal burns; and harmful temperature extremes:    On or about October 8, 2014 and at times prior thereto, at the Doming Room, the employer did not enforce the use of hand protection when employees were distributing and removing air holes from polyurethane resin coating (ingredient Isophorone Diisocyanate) that had been automatically dispensed through spigot tubing on the tops of flat pressure sensitive labels. Employee's hands were exposed to the chemical when having to wipe the chemical from the utensil in which they used to distribute the coating. This practice exposed employees to dermal sensitization and absorption hazards from the coating droplets.

29 CFR 1910.134(c)(2)(i): Respirator users were not provided with the information contained in Appendix D to 29 CFR 1910.134 when the employer determined that any voluntary respirator use was permissible:   On or about October 8, 2014, and at times prior thereto, at the facility, the employer did not provide information contained in Appendix D to employees who were given the use of filtering facepieces (dust masks) on a voluntary use basis.  Doming Technician (s) were provided filtering facepieces (dust masks) to be used for protection against Isophorone Diisocyanate vapors, while performing coating operations on the DeMak and Q-Plant machines and were not issued information contained in Appendix D orally or verbally. Dust masks do not provide protection against vapors. This practice exposed employees to not being able to take the certain precautions to be sure that the respirator itself did not present a hazard.

Employees were observed eating and drinking in areas where exposure to toxic vapors were present. This practice exposed employees to ingestion and absorption hazards associated with consumption of food while using a paper towel and bare hands to wipe droplets of Polyurethane Resin coating from the flat-head disassembly rod being used to dispense the chemical.
