29 CFR 1926.501(b)(10): Each employee engaged in roofing activities on low-slope roofs, with unprotected sides and edges 6 feet (1.8 m) or more above lower levels  was not protected from falling by guardrail systems, safety net systems, personal fall arrest systems, or a combination of warning line system and guardrail system, warning line system and safety net system, or warning line system and personal fall arrest system, or warning line system and safety monitoring system:  (a) 341 Newton St., South Hadley, MA: On or about September 12, 2014, an employee was observed roofing this building more than 19 feet above the ground below without any fall protection.

29 CFR 1926.1053(b)(1): When portable ladders are used for access to an upper landing surface, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder is used to gain access:  (a) 341 Newton St., South Hadley, MA: On or about September 12, 2014 an employee was observed using an extension ladder that did not extend the required 3 feet past the upper landing surface he was accessing.
