29 CFR 1910.23(a)(2): Ladderway floor opening(s) or platform(s) were not guarded by a standard railing with standard toeboard(s) on all exposed sides (except at the entrance to opening) with passage through the railing either provided with a swinging gate or so offset that a person could walk directly into the opening:    Employees performing maintenance or housekeeping tasks were exposed to fall hazards in that the ladderway floor openings were not guarded for the following locations:    a) The 20 X 26 inch floor opening on the east motor platform in the concrete grain elevator had a fall hazard of 23 feet to the concrete floor below.    b) The 20 X 24 inch floor opening on the west motor platform in the concrete grain elevator had a fall hazard of 23 feet to the concrete floor below.    c) The 20 X 24 inch floor opening on the east side of the boot pit in the concrete grain elevator had a fall hazard of 7 feet 5 inches to the concrete floor below.    d) The 20 X 24 inch floor opening on the west side of the boot pit in the concrete grain elevator had a fall hazard of 7 feet 5 inches to the concrete floor below.

29 CFR 1910.27(d)(2):     "Landing platforms." When ladders are used to ascend to heights exceeding 20 feet (except on chimneys), landing platforms shall be provided for each 30 feet of height or fraction thereof, except that, where no cage, well, or ladder safety device is provided, landing platforms shall be provided for each 20 feet of height or fraction thereof. Each ladder section shall be offset from adjacent sections. Where installation conditions (even for a short, unbroken length) require that adjacent sections be offset, landing platforms shall be provided at each offset.

29 CFR 1910.272(g)(1)(ii): Prior to entry, all mechanical, electrical, hydraulic, and pneumatic equipment which presented a danger to employees inside grain storage structures in the grain handling facility were not disconnected, locked out and tagged, blocked-off, or otherwise prevented from operating by other equally effective means or methods:    On or about August 28, 2014 employees cleaning out bins #1 and #4 were exposed to amputation hazards.  Locks or tags were not applied to the augers for bins #1 and #4 to prevent the unexpected start-up or energization resulting in the amputation of the toes of one of the employees.

29 CFR 1910.219(d)(1): Pulley(s) with part(s) seven feet or less from the floor or work platform were not guarded in accordance with the requirements specified in 29 CFR 1910.219(m) and (o):    Employees performing housekeeping an maintenance were exposed to amputation hazards in that the following dive pulleys were not guarded:    a) Concrete Grain Elevator East Bucket Elevator leg   b) Concrete Grain Elevator West Bucket Elevator leg  c) Steel bin grain auger across the street to the north of concrete grain elevator  d) East steel bins, bin #1 auger  e) East steel bins, bin #2 auger  f) East steel bins, bin #3 auger  g) East steel bins, bin #4 auger    The Beattie Farmers Union Cooperative Association was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1910.219(d)(1), which was contained in OSHA inspection number 314030594, citation number 1, item number 4a and was affirmed as a final order on 12/03/2010, with respect to a workplace located at 203 Hamilton, Beattie, Kansas.
