29 CFR 1910.67(c)(2)(v): A body belt shall be worn and a lanyard attached to the boom or basket when working from an aerial lift. On or about 09/18/2014, an employee was not protected from falling while working in an aerial lift approximately 20 feet from the ground level on a job site located at 4256 Cross Country Plaza, West Palm Beach, Florida.
