There was a flexible black drain pipe that was located at the top of this ladder.  Employees were using this ladder to help them access/egress their work area.

(b)  Jobsite:  On or about September 22, 2014, the employer did not instruct each employee in the recognition and avoidance of unsafe conditions when using personal fall protection.  Employees were tying themselves off with green garden hose to keep themselves from falling down the steep hillside.    (c)  Jobsite:  On or about September 22, 2014, the employer did not instruct each employee in the recognition and avoidance of unsafe conditions in how to safely reach the work area on the hillside.    (d)  Jobsite:  On or about September 22, 2014, the employer did not instruct each employee in the potential collapse of a portion of the (back porch) structure behind the house.
